Personal Records/Client Consultation forms are completed for all clients at first treatment with The Willows Therapies in my home salon or in your home.
Treatment records are kept on paper and contact details are kept on a computer, which is password encrypted to help with any changes in circumstances or issues that have happened during or after the treatment.
These personal records are kept in a locked cabinet and are held for 7 years, after this time they will be destroyed.
What we do with your data
We take this information for the following reasons:
- Internal Record Keeping
- Health and Safety
- Improving and tailor making services
This information is treated in the strictest confidence at all times
Sarah Maitland-Price and The Willows Therapies is committed to ensuring that all information given is secure.
We will not sell, distribute or lease your personal information to third parties.
However for legal reasons your data will be shared if The Willows Therapies is contacted by the Covid 19 Track and Trace team.
You have the right to ask for a copy of your personal data held by Sarah Maitland-Price on payment of a small fee which will not exceed £5.00.
